There are a few minor and almost no major changes in DSM-5 that the patients and professional need to be aware of.1. The most remarkable structural change of the Fifth Edition is getting rid of 5-axial system. Good riddance!

The old classification grouped diagnoses down into independent dimensions called axes:

Axis I: all diagnoses except mental retardation and personality disorder

Axis II: personality disorders and mental retardation

Axis III: acute medical conditions

Axis IV: psychosocial and environmental factors making things worse

Axis V: Global Assessment of Functioning (GAF), or a number between 0 and 100 that reflects patients’ well-being.

The new classification combines the axes together and let them rate the disorders by severity. In addition the NOS (not otherwise specified) label is changed to NED (not elsewhere defined).

2. The diagnosis Mental Retardation is changed to intellectual disability (intellectual developmental disorder)

3. Autism Spectrum Disorder is the new name and a single category for autistic disorder, Asperger disorder, childhood disintegrative disorder, and pervasive developmental disorder not otherwise specified (PDD-NOS). Probably a bad idea, in my opinion, as the same diagnosis will be given to a child with mild social deficit and severely autistic, nonverbal, and not functional one. In addition, it would be impossible to find out that there might be more than one disorder in this group, as all of them will carry the same name.

4. Binge Eating Disorder is a newcomer to the group – anorexia nervosa, bulimia nervosa, and eating disorder NOS – three different conditions which ended up in the Eating Disorders group only because they have something to do with food.

Ok, I have taken some time to actually read through DSM 5. While DSM has often been scrutinized–both for what it includes, as well as what it doesn’t–the back-lash against the newest edition has been particularly pronounced these past few months. Part of this stems from the micro-analysis that happens with many things in our modern world, though we have to admit that ego, resentment, and a misunderstanding of the process also plays a part. In addition, historical debate over DSM typically took place in-house; that is, by clinicians. With DSM 5, this has broadened to people who have little- to no understanding of the diagnostic process, its purpose, and its strengths/limitations. Thus, there has been a lot of negative press about DSM 5 in Huffington Post, NY Times, and other sources, often by people with practically no understanding of mental health.

Why 5 stars, you might ask? Because my review is based on DSM 5 as a book, not anything else having to do with DSM as a concept or tool. This particular edition ‘reads’ well, in that the text and lay-out is clear. In contrast to previous editions, the reader will be given more orientation to the book and how to use it. The diagnostic criteria is familiar and through the Table of Contents, Index, and quick-view pages, it is easy to find the diagnosis or category you’re looking for. Yes, it’s bulky and expensive, but you should have the large edition in your library for now; later, when you’re more familiar with the changes, you can buy the quick-reference guide.

As a child psychologist who conducts psychological and neuropsychological evaluations–for social service agencies, schools, the courts, and for families–DSM plays a prominent role in my work.

As a fan of Jodi Picoult, I was looking forward to reading her new novel. By the time I finished reading this novel, I was left with a feeling of astonishment. It is at once a storyline of such depth on the research of grieving elephants, and so rich in the storylines of those involved.

Jenna Metcalf’s mother, Alice, disappeared after a caregiver was trampled by an elephant. Jenna’s father has been in a psychiatric institution since that time, and Jenna lives with her grandmother. Jenna has not been able to forget her mother, or come to terms that her mother would leave her. Jenna does everything she can to find her mother, looking online and reads all of Alice’s old journals.

Alice was a research scientist studying elephant grief. Alice spent most of her time with the elephants that she loved. While in Africa, Alice met Thomas who ran an elephant refuge in New Hampshire. They went back to New Hampshire, married and had Jenna. All the while the love of elephants based their existence. As time goes by, Jenna is unable to find anything to support her queries about her mother. Jenna finds two people to help her, Serenity a doubted psychic, and Virgil Stanhope an older private detective. Jenna, Serenity, Thomas and Alice tell the story in alternating chapters.

Jodi Picoult does a great deal of research for her books, and with this one, she went to the Elephant Sanctuary in Tennessee, and met with Professor Abigail Baird. She also went to Vassar with Abigail Baird, who taught her all about the brain and how memory works. Then, they went to Botswana to work with Jeanetta Selier, a Ph.D. researcher who studies herds in the Tuli block. Jodi Picoult goes on to tell us about the fascinating aspects of elephants and what she learned.

I purchased this book because my four year daughter is a huge fan of the movie and to put it mildly is going through a "Frozen" phase. I thought I’d surprise her with it and it’s been worth ten times the amount that I paid for it. The only trouble is that she wants me to read it to her all the time. Or she takes it to preschool with her and wants her teachers to read it to her there. She will probably outgrow this phase soon, but for the pleasure it has given her so far it has given me a real bang for my buck!

It’s a fairly simplified breakdown of the plot of the animated film, with illustrations from the movie. Certain parts of the story are emphasized by writing words, phrases or full sentences all in capital bold letters. For example, when Anna tries to convince her sister to marry a prince she has only just met, it says:" Frustrated, Anna tried to stop her sister and accidentally PULLED OFF one glove." PULLED OFF for some reason is in bold caps while the accompanying illustration has Anna dangling the glove in the air. Every page or two have different examples of this. "Elsa had magical powers" and the "magical powers" will be the only part written in bold text but it doesn’t detract from the story. Still, the format and style are slightly unusual for a Little Golden book but like many other picture books, sometimes the text on the page will get bigger, or sometimes it will purposefully get smaller to compliment illustrations. It enthralls my little girl again and again and it also would be easy for a beginning reader to decipher.

I loved Little Golden Books as a child, and have passed many of them down to my daughter. They are all still in surprisingly good condition after 40 some years.

This is an important book in many ways. Perhaps most important because Sam Harris has, for the past several years, been a strong and outspoken critic of organized religion of all stripes. And one thing Harris can do better than almost anyone else, is make his case both clearly and powerfully without any added garbage.

If you’ve watched his many videos on YouTube, you know the man can make an argument and stand his ground without wavering one iota. And the depth of his research is impressive. If Harris kept his message in this same vein, he would stay safe and continue to be accepted as a credible spokesman for the atheist perspective for a long time to come.

But did he do that with this book? Not on your life. Harris, makes a whole different argument here, one that many may not be familiar with (but that is on display on his blog posts). Religion may be bunkum, he asserts, but spirituality (which may be the foundation of many religions), is a truly worthy pursuit.

No doubt that a great many atheists are not going to like this one little bit. After all, atheists can sometimes be as narrow-minded as believers. For many, spirituality is seen as practically equivalent to religion. But in this book he makes a strong case that nothing could be further from the truth. And he doesn’t make his arguments in a detached, completely intellectual way. Some might say that Harris has bought the spiritual kool-aid hook, link and sinker.

Harris is a long-time (25+ years) meditator, seeker after wisdom, student of a variety of spiritual practices and disciple of various teachers and gurus in several Eastern traditions. He most closely aligns himself with the school of non-duality or the direct path to awakening.

If anything uncontroversial can be said about Sam Harris, it’s that his work never fails to inspire strong and colorful opinions from just about everyone who encounters it. Depending on whom you ask, he may be one of the more brilliant thinkers around, a complete hack, or any of a mind-boggling array of subtle gradations in between. All of these views have arguable merit, and there will be many who go into Waking Up with a panoply of preconceived notions about what they might find here. Much has been made of Harris’ long-known affinity for meditation and Eastern spirituality, and his perpetual insistence that even the staunchest, most skillful rationalist neglects these at some considerable peril. Another Harris mainstay, most notably exemplified in The Moral Landscape, is a tendency to sharply challenge the conventional wisdom on where the boundaries of scientific inquiry truly lie, in what may at times strike some readers as a maddeningly quixotic attempt to reverse the long-standing unfashionable status of a rather comprehensive form of positivism. It will not shock anyone familiar with the author that Waking Up brings all of these threads together, and the reader’s satisfaction with the result, or lack thereof, will follow somewhat predictably, but it would be a mistake to avoid the book on that basis alone.

For those unfamiliar enough with Sam Harris to make much of the preceding paragraph, this volume can be summarized simply enough: it is a warning that most of us are missing important basic facts about how to live well, presented for the rationalist.

After completing the Hunger Games trilogy, I was eager for another great YA dystopian read but did not find it in The Maze Runner. I was initially intrigued by the book’s description. I knew there would be boys caught in a maze, with their memories wiped and little hope for escape, and I knew that the appearance of a girl on the scene would change everything. Mazes, games, riddles, and other sorts of non-traditional mysteries attract me, but Dashner’s execution of his book did not.

The plot was ill-paced. At times it felt slow, because Dashner introduced the reader to the maze in the same way the main character, Thomas, was introduced to it: both the reader and Thomas learn almost everything through numerous secondary explanations by characters. In more skilled hands, this might be an effective way of immersing a reader in a fictional world. Dashner’s exposition, however, felt cumbersome. As a reader, if I’m going to be told about a world rather than shown it, I’d better be told well. When I wasn’t slogging through Dashner’s writing, I was tumbling head-over-heels down its textual cliffs. Parts of the novel simply moved too quickly for any real character or plot development to occur. Readers are barely introduced to the main protagonist before being introduced to Teresa, the girl who supposedly changes everything. We really have very little sense for what’s changing, because this inciting action comes so shortly after our encounter with Thomas.

The plot also felt as if it had been constructed with little forethought. Each step or twist in the plot seemed as if it were generated on the spot as the author wrote his way linearly through this novel. Shazam! Such and such happens out of the blue. A quick patch-up of missing explanation ensues. Shazam!

A hundred or so teenage boys, their memories wiped, are trapped in the center of a gigantic shifting maze, many miles across. As the book begins, Thomas arrives in the “Glade” — the center of the maze, where they all live. The next day the first girl ever shows up too. And everything begins to change.

While living in the center of a giant ever-changing maze full of monsters is an extremely odd way to live, the boys have made do. After two years, they have a ruling council, they grow food, raise animals, and look after any sick or injured. They also send out trained runners to map the maze every day, in search of an exit, or a pattern, or some clue as to what they’re doing here.

With the arrival of Thomas and the girl, the Gladers’ carefully-crafted order begins to break down. Now solving and escaping the maze is immediately necessary. Fortunately, Thomas isn’t quite like all the other Gladers, and is able to help.

The premise is great, and the plot moves well. There’s a lot of action and the tension constantly builds. Unfortunately, the story failed in two important aspects for me.

First, the the maze itself is so absurd, the final explanation had better be pretty impressive for the story to hang together. And at least for me, the explanation was not plausible. Though, at least there *is* an explanation, which is more than can be said for some stories I’ve encountered!

The second weakness was the characters. I’d be okay with a somewhat implausible scenario if the characters were likable enough. But, Thomas is bland and whiny, and his only moments of greatness arise from his forgotten past. The other boys are mostly hostile and uninteresting. Not, mind you, that I expect deep, sophisticated personalities from amnesiac teenage boys!

The reviewers who criticize the main character’s negative tone or run-on sentences, or the lack of a cutesy, make-it-all-better ending, are missing the point of this story. The “voice” of the book is precisely why we love it so much.

Alexander is a real boy–warts and all. When real kids are upset, they pour it all out in a rapid stream of words (and to heck with grammar!)–and of course, everything feels like the end of the world to them at that moment. Judith Viorst captures that very well.

We can relate because Alexander’s life is like real life–lots of seemingly minor stressors can add up to one really rotten day; and because it isn’t just one problem, there isn’t a neat, tidy resolution at the end. In fact, in and of themselves, none of these things are really “problems”–just stuff you have to put up with sometimes. But when it all hits at once, it feels awful.

I think we’ve all had days like Alexander’s: the alarm doesn’t go off so you run out of the house late and with “bad hair,” you spill coffee on your white blouse (or new tie) just before the big meeting with the boss, you snag your nylons (or lose a button), the pop machine in the breakroom eats your money, you end up having to work overtime, so when you get out to the car you find a parking ticket because you forgot to feed the meter, and then at home, dinner burns on the stove and the kids are fighting! So at the end of it all you collapse in a heap and momentarily consider running away–FAR away. Maybe even Australia! And (adding insult to injury) nobody else seems to care or empathize, because all of these things are just little petty annoyances.
